Natural Form mattresses are designed to work on a firm, solid-surface, non-spring foundation or adjustable bed base that is structurally capable of supporting the weight of purchaser’s Natural Form mattress and user(s). This Limited Warranty and other performance warranties are based on tests conducted on “sets” that consist of our mattresses and our foundations/adjustable bed bases. THEREFORE, IF PURCHASER USES INAPPROPRIATE FOUNDATIONS, BOX SPRINGS, ADJUSTABLE BED BASES, OR BED FRAMES WITH THE MATTRESS, SUCH USE WILL VOID THIS LIMITED WARRANTY AND ALL OTHER WARRANTIES, WHETHER EXPRESSED OR IMPLIED, INCLUDING, WITHOUT LIMITATION, THE IMPLIED WARRANTIES OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE.
Natural Form may require purchaser to provide proof of the quality of the foundation, adjustable bed base, or bed frame used in conjunction with the mattress if purchaser makes a claim under this Limited Warranty. Natural Form makes no representations whatsoever as to the structural integrity or rated load for any frame, foundation, or adjustable base not manufactured by or for Natural Form. Natural Form reserves the right to invalidate this Limited Warranty if the foundation is determined, in Natural Form’s reasonable discretion, to be inadequate or if the mattress is found to be in an unsanitary condition.
The limited warranty covers the following items during normal wear:
1. Deterioration causing the mattress to have a visible indentation greater than three- fourths (0.75) of an inch that is not associated with an indentation or sag which results from use of an improper or unsupportive foundation or adjustable bed base. Normal wear requires that a purchaser’s mattress be continuously supported by a matching foundation or adjustable bed base with a proper bed frame sufficient to support the collective weight of the mattress and foundation.
A proper bed frame must provide sturdy support for both the mattress and foundation; including a center support and having at least 5-6 legs for queen, king and cal king and 4 leg support for twin and full mattresses. For queen, king, and cal king slatted style bed frames, it is incumbent upon purchaser to ensure that the frame utilized offers ample structural support for the mattress and foundation in view of that particular slatted style frame. Failure to have a proper bed frame may invalidate the limited warranty. The use of a bed frame is important for the life and protection of your sleep system. Neither mattresses, individually, nor sleep systems (mattress and foundation combination) are intended to sit/lay flat on the floor or on a waterbed frame or platform of untreated wood or untreated wood covered with fabric.
Please Note: Natural Form highly recommends a 3-bed beam support system)or a support system of similar quality that is designed specifically to give slatted style bed frames for queen, king and cal king mattresses additional structural support.
2. Any physical flaw in the mattress that causes the NATURAL FORM® material to split or crack, despite normal usage and proper handling (as outlined in the Mattress Use & Care Instructions)
3. Any manufacturing defect in the zipper assembly of the mattress cover. Natural Form may, at its own option, repair or replace the cover provided on the mattress. In the event that Natural Form replaces the mattress cover, Natural Form will replace it with the current style of cover available for the mattress, which may be of a different color or material than the original cover provided.
4. Any manufacturing defect in the air system. Natural Form may, at its own option, repair or replace the air system should it be determined that there is an issue. In replacing the system, Natural Form assures the consumer that the replacement product will be of same or improved materials, but does not guarantee color matching.
The limited warranty does not cover the following items during normal wear:
1. A normal increase in softness of the NATURAL FORM® pressure-relieving material which does not affect the pressure-relieving qualities of the mattress. (Does not apply to custom mattresses)
2. Comfort preference.
3. Physical abuse or damage to the structure and/or cover material, including but not limited to, burns, cuts, tears, liquid damage, or stains. However if there is a warranty defect not caused by the abuse or damage then the mattress would be covered under the standard warranty.
4. Damage associated with an improper bed frame, foundation, or adjustable base. See above description and illustrations for a proper bed frame.
5. Replacement of the accompanying piece in the Natural Form sleep system where only one piece is defective, unless that accompanying piece is also defective.
Foundation / Bed Frames | 1 Year Warranty
This limited Furniture Warranty extends to any Foundation and metal bed frame manufactured by Natural Form, purchased directly from Natural Form or Natural Form authorized retailers, and located in the United States (not including US territories) or Canada (each, “Furniture””).
The limited warranty covers the following items during normal wear:
This limited Furniture Warranty applies to defects due to faulty workmanship or materials, subject to the limitations described in this warranty.
The limited warranty does not cover the following items during normal wear:

- Damage caused by physical abuse, negligence, improper installation or misuse. With respect to Foundation, the foregoing includes but is not limited to structural or visual damage from using an improper bed frame or placing Foundation on the floor.
- Normal wear and tear as a result of spot cleaning or intended use.
- Damage caused by commercial use, such as in a hotel or store
- Damage caused by Acts of God or other natural forces.
- Replacement of any non-defective pieces of the Furniture (for example, if one component of Furniture is defective, then we will replace the defective component only).
- Any Furniture sold by resellers who are not authorized retailers of Natural Form.
In the event of a Defect, Natural Form’s sole and exclusive liability and your sole remedy under this limited Furniture Warranty will be, at Natural Form’s option, to provide a replacement Furniture, subject to your fulfillment of “Your Responsibilities” below.
Your responsibility
In the event of a Defect and in order to get the benefit of this limited Furniture Warranty, you must return your Furniture to Natural Form. Should shipping costs be required to return your Furniture, you will not be responsible for those costs. Evidence of a Defect and any claims must be sent to the address set forth at the end of this limited Furniture Warranty. In the event of a Defect, Natural Form will ship a replacement Furniture to you.
Replaced Furniture is subject to the same limited Furniture Warranty as the original Furniture. If you obtain replacement Furniture, the warranty term of the replaced Furniture begins from the date of purchase of the original Furniture.
Product Replacement Terms
Except as otherwise provided herein, Natural Form will not charge purchaser to repair or replace purchaser’s mattress or flat foundation if it is deemed defective during the length of this 10 year Limited Warranty, but any transportation costs associated with repairs or replacements are purchaser’s responsibility after the first year of ownership.
Adjustable Bases | 25 Year Warranty
In the event of a Defect and in order to get the benefit of this limited Furniture Warranty, you must return your Furniture to Natural Form. Should shipping costs be required to return your Furniture, you will not be responsible for those costs. Evidence of a Defect and any claims must be sent to the address set forth at the end of this limited Furniture Warranty. In the event of a Defect, Natural Form will ship a replacement Furniture to you. Replaced Furniture is subject to the same limited Furniture Warranty as the original Furniture. If you obtain replacement Furniture, the warranty term of the replaced Furniture begins from the date of purchase of the original Furniture.
1st Year Full Warranty
For the first year from the Warranty Commencement Date, your adjustable base is warranted against non-excluded defects in L&P’s workmanship or materials. During the first year from the Warranty Commencement Date, L&P will repair or replace (at no cost to You) any defective adjustable base part, and L&P will pay all authorized labor and transportation costs associated with the repair or replacement of any parts found to be defective.
2nd & 3rd Year Limited Warranty
During the second and third year from the Warranty Commencement Date, L&P will replace any adjustable base part found to be defective and not excluded by this warranty. You are responsible to pay all service and transportation costs related to the costs of receiving and installation of the new part.
4th Year To End Of 25th Year Limited Warranty
Starting in the fourth year from the Warranty Commencement Date and through to the end of the 25th year from the Warranty Commencement Date, L&P will replace steel and mechanical base parts found to be defective and not excluded by this warranty. This warranty does not cover any electronics, electrical components, drive motors or massage motors. You are responsible
to pay all service and transportation costs related to receiving and installation of the new part and a portion
of the cost of the replacement steel and mechanical
part, based on the number of months that the base has been owned by you since the fourth anniversary of the Warranty Commencement Date. By the end of the 25th year, no portion of the replacement cost is covered by L&P. You will be required to pay 1/22nd of the then current replacement cost of the part multiplied by the number of years past the third year from the Warranty Commencement Date. This portion is calculated using the following formula: Cost of the part divided by 22 [being the number of years in years 4 to 25 inclusive], multiplied by the number of years you have owned the base since
the end of the first 3 years. For example, if you make a claim for a part eligible for this warranty in the 5th year since the Warranty Commencement Date, you would be responsible for paying: $100 part cost divided by 22, multiplied by 2 = $9.09.
Additional Terms & Conditions
This warranty requires notice to be given to L&P of any claim for repairs or replacement parts.
Exclusions
This warranty does not apply; (a) to any damage caused by You; (b) if there has been any repair or replacement of adjustable base parts by an unauthorized person; (c) if the adjustable base has been mishandled (whether
in transit or by other means), subjected to physical or electrical abuse or misuse, or otherwise operated in any manner inconsistent with the operation and maintenance procedures outlined in the Owner’s Manual and this warranty; (d) to damage to mattresses, fabric, cables, electrical cords or items supplied by dealers (contact the dealer for warranty information on these items); (e) if there has been any modification of the adjustable base without prior written consent by L&P; or (f) to costs for unnecessary service calls, including costs for in-home service calls solely for the purpose of educating You about the adjustable base or finding a satisfactory power connection.
This warranty will be void if either the recommended weight restriction is not followed (refer to the advisory section of the Owner’s Manual) or if You sell or give or transfer the base to another person.
Any repairs to or replacement to Your adjustable base or its components under the terms of this limited warranty does not extend the applicable warranty from the Warranty Commencement Date. This time limitation may not apply in some jurisdictions, including the Province of Quebec.
The decision to repair or to replace defective parts under this warranty shall be made, or cause to be made, by L&P at its option and in its sole discretion.
Repair or replacement shall be the sole remedy of the Purchaser. There shall be no liability on the part of L&P for any special, indirect, incidental, or consequential damages or for any other damage, claim, or loss not expressly covered by terms of this warranty.
This limited warranty does not include reimbursement for inconvenience, removal, installation, setup time, loss of use, shipping, or any other costs or expenses. L&P and its service technicians will not be responsible for moving furniture or any other items not attached to the adjustable base in order to perform service on the adjustable base.
It is the sole responsibility of You to provided adequate space and accessibility to the adjustable base. In the event that the technician is unable to perform service due to lack of accessibility, the service call will be billed to You and the service will have to be rescheduled.
L&P makes no other warranty whatever, express or implied, and all implied warranties of merchantability and fitness for a particular purpose are disclaimed by L&P and excluded from this agreement.
Some American States do not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to You.
This warranty give You specific legal rights, and You may also have other rights, which may vary from jurisdiction to jurisdiction. This warranty is valid in all 50 American States, the District of Columbia, Puerto Rico, and all 10 Provinces and 3 Territories of Canada.
